Your answers help us prepare for your tattoo or piercing and discuss any relevant health information with you. Please read each section carefully and ask us if anything is unclear.
Choose Tattoo & Piercing Consent or Infant & Child Piercing Consent. For a child’s appointment, the form should be completed by their parent or legal guardian.
We’ll review your answers and confirm the procedure with you before we begin. Please bring valid photo ID and any documents requested by the studio.
